To start and complete a Telehealth/EAP case follow these steps and use the links provided. This simplifies the entire process making it easier for you, your group, and their employees!

Step 1

#1- Complete the Master Application.

#2- Enroll your clients through ExpressEnroll on your Enrollment Link. If you are using an Enrollment Director, please complete the FECN form after your enrollment is complete.

If you are not using your Enrollment Director, you are responsible for entering all of the client data in the approved spreadsheet. You can download the Spreadsheet here:

You will need to go to File, then Download as an Excel File for functionality

#3- Securely Transmit your COMPLETE Spreadsheet and Upload it in this Form: (Secure Transmittal Upload)
